• YOU ARE RESPONSIBLE FOR TRANSPORTATION AND POST-OP MEDICATION

• YOU MUST PAY A NON-REFUNDABLE FEE OF $50 VIA THE PAYPAL DONATION BUTTON ON OUR WEBPAGE BEFORE WE CAN GUARANTEE YOU AN APPOINTMENT.  THIS IS PAID TO US ON THIS SITE.

You must be a resident of Coos County and 18 years or older to fill out this form.  By submitting this form you are confirming that the pet listed is a personal pet.  This program is not for animals you plan to sell. 

 

We pay for the surgery only.  You are responsible for the cost of Post-Op Medication, which is $12, and will need to be paid directly to S/Nipped on the day of your appointment.  If you choose to have any vaccines or other procedures done at that time you are responsible for that cost.

 

Appointments will be on Thursdays, with an 8:30 am drop off and 3 pm pick up.  S/Nipped will contact you about the exact date.

 

PRE SURGERY INSTRUCTIONS 

No food after 9 PM the night prior to surgery, water is fine.

Dogs must be on a leash or in a carrier
